Husqvarna 128 CD string trimmer

Hello: Friend of mine has the trimmer. Apparently, the line head went bad and he ordered a replacement. And apparently it was a universal one. He called fo assistance due to the head coming off the threaded shaft as he was trimming, which was odd to me. When i checked it out, the female threads didnt look good, possibly cross threaded. The male thread on the shaft looked good but not perfect. I was able to kinda clean out the threads and i was able to thread it on the shaft, reverse threads of course. He fired it up, made one pass down the drive and all was good.

Within the hour, he text to advise the first time he bumped to advance line, the head came off rolling down the driveway! I know i personally used a wrench and secured it to the shaft. And i am pretty sure, because of the reverse threads, the head should tighten as the trimmer is spinning.

So where to i go from here? He did advise me through a photo that he ordered an OEM T25 this time. Is there a piece missing that im not aware of? Is there something that locks the strong head to the shaft? I have an older Husqvarna and i have never experienced this.

ANy direction would be greatly appreciated. Thank

Was a short universal thread adapter added to the existing original shaft end? What it the direction of rotation of the string head as viewed from the bottom looking up.... not down the shaft from the operator position?
 
A lot of universal heads come with various adapters you have to pick the one that matches the threads of the trimmer and change it out in the head.

As far as I know, there is/was no adapters at all. The line spool was threaded directly to the shaft. Looking from the bottom up, the spool screws on to the shaft counter clockwise and the motor rotates clockwise. Thanx for reply

Do believe there were any adapters. What I did was install the spool counterclockwise directly on the the units shaft. Nothing in between. Thanx for reply
