plant the tree with the top root only 1-2" below the grade. One big problem is planting too deep. After planting water it well, wait 3 days and water it again, and then every 10 days until the ground freezes. The roots need air so if you water it every day it will water log it. I don't know your soil, so can't suggest fertilizer. Sorry.
I can't recall the exact product name, but I have some liquid "Root Stimulator" I think I got at Lowes and it is specifically for transplanting shrubs & trees. I've used it and recommend it. Bone Meal is also very good for intial plantings of about anything and stimulates strong root growth.
