How do I check to see if the bar is bent? Is there an exact procedure to do this? Also why would the bar be bent?

If it's seen a lot of use, it could be a worn chain sprocket. But as essasmallengines said, my money is on a bent bar.
The bar could get bent if you pinch it in a cut and then push/pull the saw side-to-side trying to get it out. To check to see if its bent, remove the bar from the saw and sight down the length of the bar, it will be pretty obvious.
